I am looking at some pictures that a friend took on a visit that Miggy, Jackson, Santiago and Oliver all took to a hospital. Miggy does look like he has lost a little bit of weight but he is no doubt still a very big boy. I would imagine he will continue working hard if he truly wants to succeed with the swap to 3B.

The DH would be replaced with Inge or Kelly, and then Cabrera would swap positions with them.

I'm not 100% sure on this, but I do believe you could DH for any position, not just the pitcher. It does make sense, because the DH replaces only the pitcher in the batting order. You can only replace the DH with someone not in the lineup.
