Actually about everyone i know in real life cares. We always talk about how its an embarassment and that if he lost weight, he would be able to move around better. I guess you don't know anyone like that, though...yeah right OR maybe they don't talk about it around you since you're so sensitive about being fat. He is overweight, but he hits great, smart, and with power,
Could he stand to do more for conditioning/toning, yeah sure,
but will it, taking some off now, improve any of his hitting, maybe, maybe not,

A few years from now, Fielder will to need to look at his situation a bit closer, to continue his position at first base, larger players do usually lose it faster in the early 30's, than losing everything more gracefully into their late 30's. Meaning, he is a DH in 5 years or so, if he doesn't take say 20 lbs. off. jmo.
